What does a Care Coordinator do at Evercare?

Care Coordinators provide non-face-to-face care coordination services including:• Coordinating appointments and referrals

• Communicating between providers
• Supporting medication reconciliation logistics
• Managing documentation and care plans
• Following up after visits or hospital discharges
• Assisting with insurance and benefit navigation
• You will not diagnose, prescribe, or provide medical treatment.
What are documentation expectations?

Is this therapy or counseling?

No. This role is not psychotherapy. It is not outpatient mental health care.

While you may use your clinical training in communication and assessment, services are focused on care coordination and chronic care management support under CMS guidelines.

What is the compensation?

Compensation is $45 per hour.

This is a 1099 independent contractor role. Care Coordinators provide care coordination services on a contract basis. Contractors maintain professional independence and are responsible for their own licensure, taxes, and continuing education requirements.

Hours may vary based on patient volume and program structure.
